The patient and family members must understand that this is an ongoing process that begins at admission and is adjusted and monitored until discharge. It’s not an attempt to get rid of the patient as quickly as possible. Discharge planning involves continuous evaluation of the patient during the stay. Needs are evaluated and monitored up until the time of discharge. The patient’s discharge goals and intervention needs are identified and discussed. The interdisciplinary team, patient, and patient’s family are all involved in discharge planning.
